Indonesia’s journey toward strengthening its health supply chain system demonstrates the power of digital transformation in addressing long-standing challenges in healthcare delivery. Through a strategic partnership between the Ministry of Health and UNDP Indonesia, the country successfully developed and scaled SMILE, an open-source digital solution for medical logistics and supply chain management. Integrated within the broader SATUSEHAT interoperability platform, SMILE now plays a pivotal role in improving stock visibility, reducing wastage, and enhancing last-mile distribution of vaccines and medical supplies. This progress reflects the government’s strong commitment to digital health, guided by the 2021 – 2024 Indonesia Digital Roadmap and the Ministry of Health’s Digital Health Transformation Strategy, which promotes robust governance, enterprise architecture, and sustained investments across the health sector.

Indonesia’s experience offers valuable lessons for other countries navigating similar complexities in healthcare supply chains. Faced with the challenge of managing immunization logistics across its vast and dispersed geography, Indonesia tackled critical issues of data fragmentation, stockouts, and vaccine wastage that once hindered service delivery. By shifting from paper-based processes to real-time digital monitoring, the government gained greater visibility from the national level down to the last mile. As SMILE integrates further with SATUSEHAT, it strengthens Indonesia’s digital health ecosystem and paves the way for more informed decision-making, better resource allocation, and ultimately, improved health outcomes. This case study stands as a model for South-South collaboration and knowledge exchange, offering practical insights to inform national digital health initiatives worldwide.
